Once you fill out this form, we will obtain your prescription from your current pharmacy. Nothing else for you to do after that! We take care of the rest. We will call you once you are set up to schedule delivery of your medications. If you do not have medications at a current pharmacy, have a new prescription or just have questions, just leave that section blank, and we will contact you about the next steps.
Ask us about blister packs! We can set them up for you or a family member automatically every 2 weeks or monthly.
